Segue Clinic helps restore hope, healing and health to people fighting opioid addiction.

Addiction treatment works best when it engages the body, mind and social issues.

Addiction can be a chronic condition – but it can be managed by facilitating change and promoting recovery.

Successful treatment typically requires both medical management and counselling.

How We Do it

There are four phases in the treatment plan. Each patient is unique and your program length will be customized for you.

Phase One: START

At your first meeting, we will assess your level of opioid addiction and need for treatment. When you begin your program, we’ll help you to transition from using a street opioid drug (Oxycontin, Percocet, Hydromorph Contin, Codeine, Heroin, Fentanyl, etc.) to either Suboxone or Methadone.

Phase Two: STABLIZATION

You’ll have weekly appointments and drug tests during the stabilization period. You’ll receive regular counselling and meet with your Segue Clinic physician as often as needed until you are medically stabilized.

Phase Three: MAINTENANCE

During the maintenance phase, we’ll help you focus on continued participation in counselling, show you how to prevent a relapse and continue your sobriety from all illicit substances. Some people choose to continue with occasional use and if that’s your choice, we’ll work with you on a harm-reduction approach.

Phase Four: RECOVERY

In the recovery stage, you’ll become part of a new community with the skills and strength to prevent a relapse. The medication you’ll be taking will be gradually decreased over a period of several months. After the medication is stopped, we’ll encourage and help you to continue to participate in a recovery group.

 

Segue Facts


Accessibility  Since 2003, we have provided rapid access to addiction treatment for opioids (RAAM) in the community.  Our clinics assess 98% of people on the same day that they present at the clinic to see if they are eligible for our treatment programs. New patients are started on medication that same day.  Flexible hours (evening and Saturday mornings) ensure easy access our programs.
Integration  94% are connected with high-quality medical and psychosocial services such as CASON and QUEST CHC.  In partnership with different community agencies, Segue Clinic has brought the Smart Recovery initiative to the Niagara Region.  We will also connect you to the closest partner pharmacy which dispenses Methadone and Suboxone.  This partnership will ensure the best hours of access, (including on-call access between the pharmacy and the physician if needed), as well as a high quality of care and convenience
Retention  The majority of our patients not only stop using opioids, but markedly decrease their use of other drugs including alcohol.  Once stabilized and abstinent, 90% had improved housing and 80% had better jobs. Almost all the patients in remission or recovery report an improvement in social and family relationships.
Advocacy  We can advocate for you if there is a drug related justice issues.  Drug courts look favorably on participants who are abstinent and in programs. We can also, if needed, work with agencies such as Family and Children's Services (FACS). Families can remain together when abstinence is demonstrated and people in need are supported.
